At Southwest Care-Line, we believe that care should be personal, compassionate, and above all, empowering. As a family-run health and social care provider based in Somerset, we offer a range of services including domiciliary care, complex care, hospital discharge support, and supported living all tailored to the unique needs of individuals across the South West of England.

Our Story
At Southwest Care-Line, our story is both deeply personal and professionally driven. Founded by two experienced healthcare professionals, Ityai and Tutsirayi, we bring a combined background in nursing, mental health, and community-based care. Our extensive work across both clinical and social care settings has given us not only the skills, but also the insight to deliver care that is truly person-centred.

What sets us apart is the lived experience behind our mission. We have each cared for, and sadly lost, loved ones to terminal illness. We understand, first-hand, the emotional strain, the physical demands, and the small, powerful moments that matter most to families during these times. These personal journeys have shaped every aspect of our vision: to build a service that goes beyond meeting care standards, one that truly honours dignity, comfort, and individual choice.

Ityai Zembe - Director
With over 20 years of experience in nursing and social care, Ityai Zembe brings compassionate leadership, clinical expertise, and a proven track record of excellence to Southwest Care-Line. As a Registered Nurse and MSc-qualified leader, Ityai has worked across frontline care, hospital discharge planning, and health system coordination always placing person-centred care at the heart of his approach.
Ityai played a pivotal role in NHS Patient Flow Co-ordination projects, where he led hospital discharge planning and capacity management across both primary and secondary care. This work shaped his deep understanding of the challenges individuals face when transitioning from hospital to home and inspired his shift toward creating quality care capacity within the social care sector.
Since then, Ityai has successfully led multiple care providers through rigorous CQC inspections, achieving “Good” ratings and building services that are safe, responsive, and empowering. He is proud to lead Southwest Care-Line with a clear mission: to deliver care that is clinically sound, deeply respectful, and rooted in dignity.
As a qualified Oliver McGowan Mandatory Training Lead Trainer, Ityai champions inclusive, person-led support for people with learning disabilities and autism. His work reflects a strong belief that care should amplify people’s strengths and provide real opportunities to thrive no matter their circumstances.
